Guidelines for Rotary Club “Adopt a Child” Program

1. Once a patient is identified/diagnosed with hydrocephalus and proper endorsement has been obtained from a specific barangay office where the recipient lives and a certification of indigency is obtained from the local DSWD social worker, the Foundation’s office should be notified with the particulars of such patient at telephone numbers (632) 373-4084; 415-0008.

2. If the patient has not had any previous medical work up, then the patient will be scheduled for blood work, chest x-ray and CT Scan of the head. An MRI of the head maybe ordered if indicated by the attending physician or neuro-surgeon.

3. The Foundation will make arrangement for the patient to be seen by a neuro-surgeon who will evaluate and schedule the patient for shunting procedure.

4. A Memorandum of Agreement (MOA) will then be signed by the Rotary Club District Sponsor with The World Citi Medical Center with office address at 960 Aurora Blvd., Quezon City (c/o Mey Yao, 15th floor, with contact numbers  0917-5155651 or  913-8380 loc. 1501) for issuance of a guarantee memo or note to the hospital for the charges that the recipient may incur.

5. The Rotary Club District Sponsor will then pay the final hospital bill of the recipient, when patient is ordered discharged by the attending physician/ neuro-surgeon. The patient/recipient is also advised to apply for PHILHEALTH membership prior to the laboratory work-up and shunting procedure.
